If you are a new patient to the practice, you may go to the Patient Portal to complete most of your pre-registration information.  To get to the Patient Portal, first click on the following link: 

 

 

Then enter the login code provided to you when you scheduled your appointment, as well as your birthdate. 

 

Once in the Patient Portal, please enter the requested information, including your emergency contacts, individuals that may accompany you during your appointment, your medical history, medications, symptoms, and reason for your visit. When entering your medications, please enter the medication name, dose, and instructions exactly as they appear on the prescription container.

Please complete the Registration, Release of Information, Authorization for Release of Medical Records, and Pharmacy Information forms, and bring them to your appointment.
